Tipping customs vary widely by country: 15–20% is standard in the US, 10–15% in the UK and India where service charge is not already included, and 0% in Japan where tipping is considered insulting.

Convention varies. In the US most diners tip on the post-tax (final) total. In Europe and Asia where VAT/GST is included in menu prices, tipping is on the final shown total. Tipping on the pre-tax subtotal is more "mathematically correct" (you are tipping the server, not the government) but is uncommon.
Generally no. If your bill already shows a "service charge" of 10% or more, the gratuity has been included and any additional tip is optional. Note that "service charge" in India is sometimes pocketed by the restaurant rather than passed to staff — leaving cash directly with your server ensures it reaches them.
