Endotracheal tube sizes by age and weight with insertion depth.
An endotracheal (ET) tube size chart provides the correct internal diameter (ID) of the ET tube for a given patient age or weight, plus the depth to which the tube should be advanced past the lips or gums. Selecting the wrong size or insertion depth risks airway trauma, single-lung intubation, or inadequate ventilation.
For adults, standard ET tube sizes are 7.0–7.5 mm ID for women and 7.5–8.0 mm for men. For paediatric patients, multiple age-based formulas exist; the modern recommendation per APLS is: cuffed tube ID = (age ÷ 4) + 3.5, uncuffed ID = (age ÷ 4) + 4. Tube depth at the lips ≈ ID × 3.

Historical practice avoided cuffed tubes in children under 8 due to concerns about subglottic stenosis. Modern evidence (multiple RCTs since 2010) shows modern high-volume low-pressure microcuff tubes are safe at all paediatric ages and prevent aspiration and air leak better than uncuffed tubes. APLS now recommends cuffed tubes for all paediatric intubations beyond the neonatal period.
Gold standard: capnography showing a normal ETCO2 waveform across 6 consecutive breaths. Plus: bilateral chest rise, equal breath sounds in both axillae, absence of breath sounds over the epigastrium, condensation in the tube on expiration. A chest X-ray confirms anatomical depth (tip at T3–T4, 2 cm above carina). Tube depth changes with head movement — re-check after any patient repositioning.
